Deformed & Crusty looking injector - what's happening?

Hello Forum,

Gotta problem, I think?
I was replacing my injectors, and out they came one by one, then I got to the #6 cylinder, I think: 2nd from firewall - drivers side, and I am :ack::ack:

I am going to post a pic.

Any ideas as to what is going on? Or what happened?

The others were very clean, with some carbon, but not like this one.

Any advice as to what I should do right now, near short term, long term.
