Yoke Part Number

I searched the web but couldn't find what I'm looking for. I need a part number for the yoke that bolts to the rear axle pinion and connects to the driveshaft via a u-joint.

I'm fairly certain one of the ears was bent during a u-joint change and is causing a vibration.
